GREETINGS 2009 SUMMER CAMPERS AND FAMILIES!

We are so excited that you will be joining us this summer and are eagerly anticipating your arrival. These warm summer days will be here before we know it, and the campus will be abuzz with excitement. Campers will meet their bunkmates and make new friends, sign up for classes, learn camp songs, and create memories for a lifetime. Each summer we welcome a unique group of campers and counselors from around the world to share an incredible summer experience.  Whether this is your family’s first or fifteenth season with Windsor Mountain, we are delighted to have you be a part of the 2009 Windsor Mountain family.

New to the 2009 season is the use of electronic forms. We are excited about this transition as it is an excellent way to be environmentally conscious and we hope, more efficient. THE FOLLOWING FORMS ARE FOR ENROLLED CAMPERS ONLY. Click here for a checklist for your own use in tracking your forms.

Included below are links to much of what you will need to prepare for the start of camp. We recommend creating a folder on your desktop to save these documents. Printing is not necessary for all the documents as many are just for your reference. If you do need a paper copy of one of the forms or are unable to print them, please email Neesa@WindsorMountain.org and we can send you one.
You can fill out these forms electronically on-line, if you choose. You'll need Adobe Reader (click here for free download) , which will allow you to digitally sign the forms, then create a PDF copy of your filled out form and email them back to us as attachments. Or if you prefer, you can print them out, sign and mail or FAX them in.

2009 Health Form Family section (p1-3)
To be filled out by a Parent/Guardian. Please read the important instructions on the first page of the health form and schedule an appointment for your child’s physical now if you haven’t already done so.  Before returning the health form it must be signed by a parent/guardian, and include a photocopy of your child’s insurance card. 
2009 Health Form Physician section (p4)
This page is for your child's physician to fill out and sign. This page, or an equivalent provided by your physician containing all requested information must be signed by a physician, include all up-to-date immunization information.

2009 Summer Camp Transportation Form
Included are directions for flying, taking our chartered bus, taking our van, or driving.  PLEASE fill out and return this form EVEN IF YOU ARE DRIVING your child to camp! We need to arrange the charter buses from Manhattan, Mt. Kisco, and Hartford EARLY in the spring, and it is difficult when we don’t know how many campers are planning to take the bus.  Space on the Boston Van is very limited so the sooner you send in your complete form the easier it is to confirm your child’s seat in the van.

PLEASE NOTE: If you prefer, you may pay your Transportation Fee through PAYPAL. You do NOT have to LOG IN or open an account with Paypal in order to use this feature. You can simply make a payment to Windsor Mountain using your credit card. (When you get to the PayPal information   page, skip over the "PayPal Checkout" section, and just fill out the mandatory fields only.)

2009 Camper / Parent Questionnaire
This form is designed to give our counselors important information to guide campers as they choose their activities during the summer.

2009 Program Contract
If you applied online you and your camper must sign the 2009 Program Contract.

Vacation Form
This form provides us with your contact information if you plan to be on vacation while your child is at camp.

2009 Teacher Reference (new campers only)
This is for NEW CAMPERS ONLY, and helps us to provide a positive, creative learning environment for your child.

Camp Insurance
We want to help you protect your summer camp investment and encourage you to consider camp insurance.  It protects you in the event that your child has to cancel before the start of the program, or needs to leave camp part way through. For those of you who also attend Family Camp (Aug 22-26), this is a new, on-line application form for you to fill out.
